Drawing on evidence from cognitive science and clinical medicine, we look at how patterns like anchoring, confirmation bias, availability bias and overconfidence can drive over‑testing, financial toxicity and therapeutic “stack creep” while diverting attention from simple, high‑yield foundations.</p><p>I introduce a practical, “foundations‑first” pyramid to help practitioners sequence care from context and core physiology through to targeted hypotheses and, only at the top, advanced testing and protocols.
